We are seeking a Peer Certified Opioid Outreach Specialist to join our Adult Jail Transition program!

This position works with adult individuals who have been identified as having issues with Opioid abuse and dependence. Position works within an Inner-departmental collaboration as part of a multi-disciplinary team between the Criminal Justice Engagement Team (CJET) and the 7th Ave IOP Opioid Services program to actively guide, navigate, and refer persons into Medication Assisted Treatment Programs. Position works independently in the community, carries a caseload, and provides an appropriate level of service to the program and individuals to complete a successful transition into services, reduction of recidivism, and increase in support systems. Position seeks program candidates through doing Jail In-Reach services, by working with court contacts, SB&H hospital co-location and navigation programs, and other community providers. Provides navigation services with short term warm hand-off, individual 1:1 services, fund type enrollment, assessment, integrated treatment planning, case management, crisis and behavioral health interventions/services and supportive Peer services.


  • Provides extensive community outreach work based on the needs of the team and individuals served in multiple locations.
  • Works well independently in the community managing time and appointments as required by the program and service needs. Meets and balances multiple demands.
  • Develops working rapport with Persons Served as a priority of service
  • Acts as a Navigator and Case Manager, brokering, planning, and referring persons into Medication Assisted Treatment and other community services using an integrated approach.
  • Provides inter-rim short term services as need to facilitate transition
  • Is familiar with various funding mechanisms as to enroll the client in services and benefits in accordance with needs.
  • Completes comprehensive assessments, intake and enrollment paperwork in a timely manner
  • Participates in and writes Integrated Treatment planning and interventions.
  • May work to give input into pre-release planning in the jail or court system
  • Provide jail visits to work with persons served providing relevant treatment options.
  • Plans for, coordinates, and assists client with immediate and basic needs required to participate in services.
  • Collaborates with Internal Programs working with multiple area supervisors; 7th Ave IOP, OP Clinics, the CRC, CJET, Hospital Programs, and other relevant programs/ navigation and outreach staff
  • Works with Maricopa County Corrections inside the jail, County and City Court Systems, and other Community Public and Provider Services
  • Required to provide group treatment and present community education as assigned.
  • Provides crisis intervention, de-escalation, and Behavioral Health interventions
  • Completes excellent clinical documentation and billing as required in the electronic health record in a timely manner. Maintains a standard productivity as set by the program.
  • Document activities for tracking and reporting according to documentation standards. May be required to document in multiple systems.
  • May be required to work flexible hours as needed to best serve the program and client needs

Education & Qualifications:

  • Minimum requirement consists of a 21-year age
  • A High School Diploma or GED plus some college course work in Behavioral Health or related field work equivalent to a bachelor’s degree or 5 years’ experience.
  • Must be Peer Certified. Knowledge of Opioid Addiction personal or professional preferred
  • Must quality to drive company vehicles with valid AZ driver’s license and clean MVR.
  • Must be able to pass a criminal background check and qualify for a Jail access badge
  • Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire
  • Ability to work within multiple different systems with varying policies and procedures.
  • Ability to work long hours in the community and navigate changing daily demands
  • Bilingual (English/Spanish) speaking preferred.
