Job Description
Pay rate range between $26.29 - $31.00 hourly
Schedule: Tuesday – Saturday 7:00 AM - 3:00 PM or Sunday – Thursday 7:00 AM – 3:00 PM
Work as part of a clinical multi-disciplinary team. Provides medication assistance to consumers residing in a Community Living setting.
Department Goals:
- To provide timely Recovery Based Clinical and Non- Clinical supports services to adults.
- Integrate quality assurance/clinical excellence processes to provide the safe delivery of quality Behavioral Health care within reasonable financial resource limits.
- Develop and maintain sound clinical programs that promote community and independent living.
- To enhance SBH’s community image, community relations, and consumer confidence. Review and analyze aggregated data in order to help enhance the overall performance of the department
Job Duties:
- Monitor and educate clients on known health conditions such as diabetes, high blood pressure, and obesity.
- Assist consumers in self-administration of medications in accordance with provider orders
- Review the effects and side effects of medications.
- Improve medication adherence
- Provide monitoring of physical/medical status and notify appropriate medical personnel with concerns
- Accepts medication orders from physician via telephone and medication records in accordance with State regulations, indications and resources.
- Works with prescribers in the monitoring of medications and changing of medication orders.
- Completes charting and record keeping per SBH/ADHS standards.
- Participate in Clinical case staffing.
- Facilitate coordination with the consumer’s clinical team and PCP, and any specialists as needed.
- Assists in SBH chart audit process.
Qualifications:
- Must be a LPN, currently licensed in the States of Arizona. Must have post-degree experience of two years in the area of behavioral health and/or healthcare nursing. Motor Vehicle clearance required.
- Experience in a community based psychiatric practice preferred.
- Experience with SMI population preferred but not required
