We are seeking a Housing Program Coordinator for the HOPWA Program

General Description:

Responsible for the ongoing coordination and development of a housing program for individuals living with HIV/AIDS. Functions are to maintain program, operate at maximum capacity, and fulfill contract requirements.  Facilitates eligibility screening and housing placement for individuals/families infected and affected with HIV/AIDS. Assists residents in achieving housing stability within a continuum of housing services. Functions as an integral part of an interdisciplinary team to develop individual housing plans. Responsible for contributing to and creating a recovery based environment.  Performs networking and marketing functions to build program areas and program revenues.

Duties & Responsibilities:

  • Works with the City of Phoenix and other funding sources to ensure proper reporting is complete and accurate to maintain HUD funding

  • Creates implements and monitors an action plan which identifies the resident’s areas of strength and challenges, establishes goals and outcomes, keeps resident file documentation up to date, monitors resident’s progress, provides for necessary follow-up and referral at case closure.

  • Coordinate with JRC Program Coordinator and staff to ensure coordination in services for HOPWA clientele.

  • Performs intakes, screening and assessments on potential residents; identifies most beneficial placement.
  • Plans and provides limited direct services for residents of the program.
  • Provides services to permanent housing residents both individual and families; oversees temporary housing services.
  • In conjunction with the Housing Specialist will provide housing support, as needed.

Intervenes in crisis situations as needed and coordinates with the support/crisis team.

  • Identifies unmet needs of residents, coordinates referrals and provides follow-up.

  • Links to community resources, particularly for recreation, education, medical, vocational purposes.
  • Provide coordination of care with external providers to include specialized care in crisis management, vocational services or other specific needs of the program and/or population.
  • Attends and participates in in-service and related training opportunities as available and appropriate.
  • Documents resident services in accordance with approved clinical standards of SBH.
  • Develops and implements policy and procedures for staff and program as needed.

  • Recruits, trains, and supervises staff, interns and/or volunteers for the program.
  • Oversees hygiene and safety standards of all units; monitors inspections of apartments, smoke detectors and fire extinguishers. Reviews emergency escape plans and general unit up-keep with residents and/or housing specialists.
  • Responsible for all aspects of the housing plans including statistical and data collection reports and outcome measurements and other reporting required by SBH and program funders.
  • Performs consultation/education activities with other agencies or community groups when appropriate.
  • Serves on SBH committees when requested by supervisor.
  • Maintains and purchases program supplies and monitors program budget.
  • Understands Arizona Landlord and Tenant Act and its application to residents and units.

  • Calculates and collects program fees.
  • Participates in community planning, volunteering, advocacy, and networking with other providers and/or funding sources, as directed by supervisor.
  • Responsible for fulfilling grant and/or funding requirements including but not limited to:  competing grant applications/renewals and compiling reports/data.
  • Ensure CONFIDENTIALITY in all areas of service corporate wide.

To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.

Education & Qualifications:

 Minimum requirement consists of a 21-year age, Bachelor’s Degree and two years of experience (or equivalent education/experience) with at least a minimum of 2 years supervisory experience in Behavioral Health, preferably in a community living/residential setting/ and/or HIV/AIDS Community.

 Minimum requirement consists of a 21-year age, Bachelor’s Degree and two years of experience (or equivalent education/experience) with at least a minimum of 2 years supervisory experience in Behavioral Health, preferably in a community living/residential setting/ and/or HIV/AIDS Community.
