We are seeking a Community Outreach Navigator to join our Adult Jail Transition Program!

This position will work with adult individuals who frequent the legal system and/or are transitioning within or from jail or prison systems. It works as part of a multi-disciplinary team with the City, case managers and RBHA to successfully transition persons from jail or prison to services and community stability. Position works independently, carries a caseload, and provides an appropriate level of service to the program and individuals to complete a successful transition, reduction of recidivism, reduction of incarceration, and increase in support systems.

Responsibilities:

  • Works in cooperation as part of a multi-disciplinary team with City, Police, Jail/Prison Systems, Prosecutors, Court System, PNO Case managers, and Program Liaisons
  • Provides extensive outreach work based on the needs of the team and individuals served.
  • Responsible for taking the lead in coordination and facilitation of Jail/Prison transition
  • May require work inside the jail and community 
  • Works with the Prosecutors Office to develop terms for release
  • Provide jail visits to meet and prepare the members for court.
  • Reviews with client the expectations upon release and follows up as needed.
  • Act as an Advocate in court.
  • Plan, coordinate, and assist client with immediate and basics needs for release.
  • Make referrals to appropriate services in cooperation with PNO clinical teams, and monitor members participation & compliance.
  • Works to provide and or facilitate the prescribed treatment planned accordingly
  • Coordinates and participates in staff meetings and member staffing’s.
  • Develops working rapport with team, persons served, legal system, PNO clinical teams as to effectively perform all job functions.
  • Works with persons to reestablish benefits
  • Responsible for case management and carries an independent case load.
  • Follows up on all referrals in a timely manner.
  • Provide screenings & and ongoing assessments.
  • Provides crisis intervention, de-escalation, and Behavioral Health interventions.
  • Assist in facilitating needs to help individuals be successful in the program.
  • Document activities for tracking and reporting according to documentation standards
  • Responsible for contractual obligations, fulfillment and outcome measures.
  • May be required to work flexible hours as needed to best serve the program and client needs
  • Required to carry a company smart phone
  • Required to drive personal or company vehicle and keep appropriate documentation.
  • Must qualify and maintain in good standing; a Jail access pass.
  • To ensure compliance & adequate services, additional job duties may be required and assigned.

Education & Qualifications:

  • Minimum requirement consists of a 21-year age, High School Diploma or GED plus some college course work in Behavioral Health or related fields, plus one year directly related job experience, or the equivalent combination of experience and education as well as Motor Vehicle clearance.

  • Maintain a Valid Arizona Driver’s License and a clean driving record for last 39 months is required. Three years of driving experience is required.

  • Peer Support Certificate preferred

  • Must have fingerprint clearance issued by Arizona Department of Public Safety or eligible to receive within eight (8) weeks of hire. 

  • Eligible to receive clearance for Jail visits.

  • Experience working with multi-cultural and low socio-economic individuals. General knowledge and aptitude for working independently with adults of mixed ethnic groups. Bilingual (English/Spanish) speaking preferred.
