We are seeking a Children’s Case Manager for our Prescott Valley Outpatient Clinic! This is a full time, 40 hour per week position. Work hours will be Monday – Friday between 8 am to 7 pm, based on client and clinic needs. Flexibility in work schedule is a must.
In this position you may provide direct services in the home or school settings. Interventions may be 1:1 and group based. Groups will primarily focus on living and recovery skills. In addition this person will performs crisis stabilization functions, assist consumer in resourcing with the community, provide vocational services, conduct assessments and complete necessary paperwork and serves as central support for GMH, SA, SMI adults and/or children and their families. Case management functions also include monitoring mental status of consumers and responding to treatment plans through face-to-face contact. Employee must ensure confidentiality of verbal and written consumer information.
Minimum requirement consists of a Bachelor’s degree with one-two years of full-time behavioral health experience, plus valid driver's license and motor vehicle clearance. Experience helping youth highly preferred . An equivalent combination of education and experience totaling five years is acceptable. Case Managers are expected to have their own reliable transportation to get to and from home visits in case the company car is being used by a fellow employee.
