We are seeking a Children’s Case Manager for our Prescott Valley Outpatient Clinic! This is a full time, 40 hour per week position.
Work hours will be Monday – Friday between 8 am to 7 pm, based on client and clinic needs. Flexibility in work schedule is a must.
In this position you may provide direct services in the home, in clinic or school settings. Interventions may be 1:1 and group based. Groups will primarily focus on living and recovery skills. In addition, this person will perform crisis stabilization functions, assist consumer in resourcing with the community, provide vocational services, conduct assessments and complete necessary paperwork and serves as central support for children and their families. Case management functions also include monitoring mental status of consumers and responding to treatment plans through face-to-face contact, facilitating regular CFT’s, attending Family Treatment Court and working with DCS with mutual clients, coordinating with juvenile probation, completing Birth-5 observations and assessments, and submitting referrals for other services as needed. Employee must ensure confidentiality of verbal and written consumer information.
- Preferred: Bachelor’s degree with minimum of 1 year of full-time behavioral health experience, plus valid driver's license and motor vehicle clearance. Experience helping youth highly preferred.
- An equivalent combination of education and experience totaling two years is acceptable.
- Case Managers are expected to have their own reliable transportation to get to and from home visits in case the company car is being used by a fellow employee.
- Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.
