We are looking for a Benefits and Housing Specialist to join our Metro Outpatient Clinic!
Job Preview at a Glance:
The goal of the Benefits and Housing Specialist is to provide one-on-one & group interventions, crisis stabilization, access to resources with the community, and care coordination. This individual will assist with referrals, coordination, admission, discharge, and out of home placements.
Responsibilities:
- Reviews documents received and requests required documents to appropriate level of care for members.
- Assesses members in accordance with established policies and procedures and clinical protocols.
- Develops service plans with members based on assessment and diagnostic process.
- Develops and maintains appropriate level of relationships with members and formal/informal support systems.
- Monitors mental status of members and response to treatment plans through face-to-face contact, and updates treatment plans as needed.
- Works collaboratively with treatment team and all external members of member’s services.
- Attends weekly clinical staffing and all staff meetings.
- Completes outreach for members who miss an appointment and follows up with re-engagement process.
- Maintains quality and compliance in members’ charts. Provides information to members and family members or guardians concerning community resources and linkages to other services.
- Follow-up with members on the HEA report and complete deliverable for Buckeye and Metro.
- Meet with members to apply for AHCCCS, Social Security, and Disability Benefits.
- Meet with members to provide additional resources regarding benefits.
- Assist non-title members with applying for AHCCCS when appropriate
- Alert members when they need to renew AHCCCS and assist
- Communicate with the clinic, members and insurance regarding benefits.
- Assist members with finding appropriate housing.
- Assist case managers with submitting housing applications for members.
- Submit referrals for TLP, BHRF and other placements as needed.
- Check on housing application status and waitlists for members.
- Complete housing deliverables for Buckeye and Metro SMI HH
Qualifications:
- Minimum requirement consists of a 21-year age
- A Bachelor’s degree with one year of full-time behavioral health experience. A combination of experience and education totaling 3 years is acceptable.
- Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.
